恶性肿瘤患者外周血淋巴细胞 mdr21 基因表达在原发耐药与继发耐药

摘要: 目的: mdr21 基因是肿瘤多药耐药现象中的主要表型 ,探讨 mdr21 基因原发耐药与继发耐药表达状况 ,预期指导临床个体化疗。方法:应用 RT2PCR 方法对51 例恶性肿瘤患者外周血淋巴细胞 mdr21 基因进行定量体外诱导后检测其表达水平。结果:51 例外周血淋巴细胞中原发性阴性表达 34 例占66% ,继发性阴性表达仅5 例占9. 8% ,外周血中的原发mdr21 阴性表达与继发性表达比较有显著性差异P < 0. 01。在低阳性组,原发性表达 11 例占 21 % ,继发性表达 31 例占 60.7 % P < 0. 01 。高阳性组原发性表达6 例占 11. 8 % ,继发性表达 15 例占 29% P < 0. 01 。继发性表达率明显高于原发性。结论:RT2PCR 检测人外周血淋巴细胞 mdr21 基因表达取材方便 ,结果可靠,提示不同程度的 mdr21 基因表达水平可以客观地反映不同个体的药物耐受情况. 为临床个体化疗提供指导性信息。

Abstract: Purpose : To probe into the inherent and acquired expression of mdr21 gene on drug2resistance. Methods : Reverse transcription2polymerase chain reaction ( RT2PCR ) assay was used to detect the inherent and acquired expression of mdr21 in the peripheral lymphocytes of 51 patients with malignant tumours. Results : There were 66 % ( 11/ 51) inherent and 9. 8 % (5/ 51) acquired expression in negative groups , and 21 % ( 11/ 51) inherent and 60. 7 % (31/ 51) acquired in low positive groups , and 11. 8 % (6/ 51) inherent and 29 % (15/ 51) acquired expression in high positive groups. Obvious differences were demonstrated among the three groups ( P < 0. 01) . Conclution : The results showed that the mdr21 expression level can reflect the situation of drug2resistance. The method using RT2PCR assay to detect mdr21 expression in the peripheral lymphocytes is convenient for selecting sensitive drugs.
